Adding a Question

Click on the ‘Add Question’ button to add a question to your survey. A window will appear allowing you to add your question, determine the answer types and set whether your question is required, optional, or dependent (see below for descriptions of each).

• Required- Member must answer the question to move on to the next.
• Optional- Member can choose not applicable if the question doesn’t apply to them.
• Dependent- Whether a member must answer this question is dependent upon another question. For example, if the member chooses Yes to Question #1, they may have to give an explanation in this question. If the member answers No to Question #1, this question will be marked automatically as Not Applicable.
